Record History: See What Changed in a Patient's Record (Early Access Only)

Learn how to use Record History in SOAP 2.0 to see who changed what in a patient's record, and when.

What is Record History?

Record History lets you see a timestamped log of changes made to a patient's SOAP 2.0 record — including who made each change and when. It's available to any team member with the View Record History permission, and is most useful for vets, nurses, and practice managers who need to review or verify record edits.

Where to Find It

Record History is available on records created in SOAP 2.0 format. It is not available on records in the previous format.

  1. Open a SOAP 2.0 patient record from the Health Card, Flowboard, or Calendar

  2. Inside the record, select Record History in the Record Navigation Bar

How to View Record History

  1. Open any SOAP 2.0 patient record

  2. Select Record History

  3. The history log will display all tracked changes in chronological order

  4. Each entry shows:

    • The field that was changed

    • The previous value and new value

    • The team member who made the change

    • The date and time of the change

What's Tracked in V1

Record History currently tracks changes to the following fields:

  • All internal record fields

  • All custom record fields (New Custom Fields)

  • Assigned vets

  • Visit Types

  • Selected diagnoses

‼️Coming soon: A future update planned for Q2 2026 will expand history tracking to include widget-level changes within a record.


Tips & Best Practices

  • Use Record History before contacting the support team — most "who changed this?" questions can now be answered directly in the record

  • Review Record History as part of your compliance or audit workflow to verify record accuracy

  • If a team member doesn't see the Record History option, check that the View Record History permission has been assigned to their role under Administration → Manage Records

  • Record History is only available on SOAP 2.0 records — records in the previous format do not include this feature

  • Record History is read-only — no changes can be made from this view

Common Questions

Why can't I see Record History in a record?

There are two possible reasons. First, Record History is only available on records in SOAP 2.0 format — it is not supported on records created in the previous format. Second, access is controlled by the View Record History permission under the Manage Records category in Administration. Ask your practice manager or administrator to verify your role's permissions.

Does Record History show changes made before this feature launched?

No. Record History tracks changes from the point this feature was enabled for your clinic. Changes made prior to activation will not appear in the log.

Does Record History show changes to all sections of the record?

V1 tracks changes to core record fields, custom fields, assigned vets, visit types, and selected diagnoses. Widget-level history is planned for a follow-up release in Q2 2026.

Can I export or print the Record History?

Record History does not currently support an export function. If you need a record of changes for compliance purposes, you can capture a screenshot of the history log.
