How to fill in Forms and Select Services When Booking an Appointment Online (beta)

When booking an appointment online through the clinic's public calendar or pet parent app, you can ask pet parents to fill in important forms and choose optional additional services.
This helps the clinic prepare for the visit and makes the check-in process smoother.

Steps to book a visit type that has connected forms and services

  1. The pet parent selects a Visit Type.

  2. Then selects the date and time for the visit.

  3. If they are not logged in, they will be asked to do so before proceeding to the next step.

  4. The pet parent arrives at the Configure step where they will be prompted to fill in the forms and (optionally) select additional services for their visit.

  5. Complete Required Forms

    • Some forms will require a signature.

    • Others may only need to be read and confirmed.

    ➔ The pet parent must open and confirm/sign all required forms before moving to the next step.

  6. Select Additional Services (Optional)
    If available, the pet parent will be able to click on the "Select additional services button". This will open up a modal that will display the services associated with the visit type.

    If there are a lot of services the pet parents are able to search for the one they are looking for by using the search field.

    The pet parent can select one, multiple, or none by clicking on the select button.
    The pet parent canclear those services from the appointment request by clicking on the "Clear all" button.

  7. Submit the Appointment Request
    Once the forms are completed and services (if any) are selected, the pet parent can submit their appointment request.


✅ What Happens After the pet parent submits the appointment

  • Your appointment request will include:

    • The completed forms that have been either signed or confirmed

    • Any additional services they have indicated
