What is Record History?
Record History gives you a complete, timestamped log of every change to a patient's record, including every widget, not just the notes. It's available to any team member with the View Record History permission, and it's most useful for practice managers, vets, and techs who need to confirm who changed something and when.
Because the history is retroactive, it covers every existing record from day one. It isn't limited to changes made after you turn on the feature for your clinic.

Where to find it
Open a patient record from the Health Card, Flowboard, or Calendar
Click the clock icon in the record toolbar, next to the record status, in the top right
This opens the Record History Panel.
How to view Record History
Open any patient record
Click the clock icon in the record toolbar
The Record History panel lists every tracked change, newest first by default
Each entry shows:
A section tag showing which part of the record the change came from: the record itself, or one of the 12 tracked widgets
An action badge: green Added, red Removed, or neutral Modified
The team member (user) who made the change, with their avatar
The date and time of the change
An old-to-new diff for any field that changed, shown in plain language: names instead of IDs, formatted dates, and units or currency on both sides
Filter and sort entries
Use the filter bar to narrow the list by:
User: who made the change
Date: when it happened
Action: Created, Modified, or Deleted
Section: the record or any of the 12 tracked widgets
Use Sort to switch between Newest first and Oldest first, independent of your filters.
What's included in the audit trail
Record History covers the record itself (template fields and record status) plus 12 widgets: Vitals, Anesthesia, Dental Chart, Medications, Treatment Plans, Services & Products, Payment, Body Maps, Certificates, Files, Tasks, and Follow-ups.
Most widgets track both entity-level actions (created, modified, removed) and individual field-level changes. Body Maps, Certificates, and Files track entity-level actions only.
Tips & best practices
Use Record History before contacting support. Most "who changed this?" questions can now be answered directly in the record, for any widget, not just the notes
Filter by Section: Payment when investigating a billing discrepancy, or by Section: Treatment Plans or Medications to confirm what changed before a dispute or shift handoff
Review Record History as part of your compliance or audit workflow to verify record accuracy
If a team member doesn't see the clock icon, check that the View Record History permission is assigned to their role under Administration → Manage Records
Record History is read-only, so no changes can be made from this view. There's no free-text search across entries either, so use the filters to narrow down what you're looking for

Why can't I see Record History in a record?
The View Record History permission, under Manage Records in Administration, controls access. Ask your practice manager to check your role's permissions.
Does Record History show changes made before this feature launched?
Yes. Record History is retroactive. It covers the full change history for every existing record, not just changes made after the feature is enabled for your clinic.
Does Record History show changes to all sections of the record?
Yes. It covers the record itself plus all 12 tracked widgets: Vitals, Anesthesia, Dental Chart, Medications, Treatment Plans, Services & Products, Payment, Body Maps, Certificates, Files, Tasks, and Follow-ups.
Can I search Record History by keyword?
No. Record History supports structured filtering by User, Date, Action, and Section, but not free-text search across entries.
Can I export, print, or undo a change from Record History?
No. There's no export or print function, and the log is read-only, so there's no rollback or restore either. If you need a record of changes for compliance purposes, take a screenshot of the history log.
