Overview
You can now associate forms and services with specific visit types. This allows you to streamline client intake, collect signatures ahead of time, and offer additional services during the online booking process.
Once configured, pet parents will see and complete the assigned forms and (optionally) select services when they book an appointment through the Public Calendar or Pet Parent App (coming soon).
Currently, forms and services are only available when booking through the Public Calendar. Support for this feature in the Pet Portal is on the way and will be available soon!
This update helps clinics:
Collect important information and signatures before appointments.
Offer relevant additional services during booking.
Streamline client intake and reduce staff workload.
✏️ How It Works
When pet parents request an appointment online:
They will be prompted to fill out any required forms (e.g., consent forms) associated with the selected visit type.
They will have the option to choose additional services related to the visit (e.g., nail trims, ear cleanings for surgery appointments).
Finalized forms and selected services will be:
Visible in the Appointment Request
Accessible from the SOAP if the SOAP is opened from the appointment card.
🛠️ How to Set Up Forms and Services for Visit Types
Configure forms
Go to Administration → Visit Types.
Click on the "Edit" icon to edit the visit.
Click on "Add new form" button.
Choose one or more forms you want to require for that visit type.
Tip: Forms must be created beforehand in Administration → Certificates & Forms.
Save your changes by clicking the "Confirm" button.
✅ Now, whenever a client attempts to book this visit type, they’ll be prompted to complete and sign the form(s) before submitting their request.
✅ At the same time, when clinic staff adds an appointment for that visit type in the public calendar, the associated form will be automatically added to the appointment.
Configure services
Go to Administration → Visit Types.
Click on the "Edit" icon to edit the visit.
Click on the dropdown below "Suggested Services for Online Bookings."
Choose one or multiple services you would like to inform the pet parent about.
Tip: Services must be created beforehand in Administration → Services.
Save your changes by clicking the "Confirm" button.
✅ Now, whenever a client attempts to book this visit type, services will be visible to the pet parent and available for selection.
Pet parents will not be required to pay for the services they select during the appointment booking process.
Pro Tip
You can choose to display/ not display the service price to the pet parent in the public calendar by checking/unchecking the "Show service price in the public calendar" preference.
This preference can be found in Administration>Clinic Profile>Preferences, Appointments section.
The appointment request
The appointments requested by the pet parents will display the forms with it's status (E.g "Signed), as well as any services they might have indicated.
Adding an appointment that has connected forms in the internal calendar
The visit type> form connection works in the public calendar as well.
When choosing to add an appointment for a visit type that has a connected service, you will notice that the form has been automatically added to the appointment.
Saving the appointment will automatically include the form in the new appointment notification email.
FAQ
What happens when the pet parent wants to book appointments for multiple pets?
They will need to fill in forms for each pet. Service selection is also available for each pet.
How many forms and services can be connected to a visit type?
As many as you want. However, bear in mind that too many forms to fill out might prevent the pet parent from completing the appointment request. Connect forms where is makes sense!
